More than ever, patients want to recover and return home. With 10,000 baby boomers turning 65 every day, demand is only growing. At the time of hospital discharge, too many people are sent to skilled nursing facilities for non-medical reasons, or sent home with inadequate care, only to quickly return to the ER or hospital. This failure stems from misaligned care models, poorly coordinated post-acute services and a lack of focus on quality. This system doesn’t work for patients now or in the future. It affects our parents, grandparents, friends and, one day, us. It’s what has led OneHome to change the face of post-acute and home-based care.
OneHome and its health plan partners recognized early on that a new model designed to get patients on the road to healing had to be built. Post-acute care needed to be reimagined to meet expanding patient needs, tied to total cost-of-care goals. We developed an integrated care, network engagement model and technology platform that solves for the new challenges, validated by our value-based care delivery to more than 5 million patients. We have met the future with a team, culture and solution that helps patients heal and get back home.
